Job Summary:
The Upstate Neurological Institute Neurotrauma RN Navigator (UNI NTNN) functions as a member of an interdisciplinary care team, providing expert care coordination related to Neurotrauma prevention, traumatic injury, and rehabilitation. The UNI NTNN is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decision making related to Neurotrauma and Neuro recovery. The UNI NTNN assumes responsibility for the coordination of all resources necessary to provide care for UNI centers of excellence. Utilizes advanced expertise to identify, implement, and improve processes to provide an integrative model of care to the patients, while assuring the highest level of patient satisfaction. The UNI NTNN provides patient education for each patient regarding their diagnosis/injury and treatment throughout the health care continuum. Demonstrates and promotes professional development. Maintains a professional, safe, and therapeutic environment for rendering care. The UNI NTNN is an advocate for the patient and their family and acts as a liaison for resources and services, navigating patients through a complex care system. Serves as a clinical resource and consultant to members of the healthcare team. The UNI NTNN will provide patients and their families neurological/surgical health education, screen adult neurological patients for psychological sequelae after diagnosis/injury (referring any positive screen to a psychologist), enroll patients into peer support networks, and provide resources specific to patient needs.
Minimum Qualifications:
NYS licensed/registered (or eligible) RN required. Bachelor's degree in Nursing or healthcare related field, three (3) years post licensure RN experience (preferably in specialty) OR an Associates degree in Nursing and five (5) years post licensure RN experience (preferably in specialty) required. Excellent written/oral communication and interpersonal skills required. CPR-BLS certification is required by the time of appointment.
Preferred Qualifications:
Three (3) years as a Certified Nurse/Patient Navigator in a specialty area preferred. RN experience with neurotrauma patients preferred.

Monday-Friday, Days
