Job Summary:

SUNY Upstate Research Foundation is seeking a highly motivated Advanced Practice Provider who will serve in the Outpatient Clinic working with the Urology Team providing care to patients at the 550 Harrison Urology Office. 

Responsibilities will include:

• Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing.
• Evaluating patients, ordering appropriate laboratory testing & diagnostic studies
• Provide outpatient, inpatient and urological procedures, arranging for referrals, testing, consultation & therapeutic services.
• Maintaining outstanding relationships with families, referring physicians & all members of the health care team.

As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.

Minimum Qualifications:

• Master’s Degree as a Nurse Practitioner or Physician Assistant.
• NYS Licensed/Registered NP or PA.
• Current National Certification consistent to the clinical practice. 
• NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
• CPR-BLS Certification required at the time of appointment.
• Must successfully pass pre-employment and credentialing clearances. 

Preferred Qualifications:

Prior experience in specialty area preferred.

Work Days:

Monday - Friday, Days. No Weekend or Holidays.
