The Social Worker will work at the Upstate Joslin Pediatric Clinic at the Nappi Wellness Center. The Social Worker will provide comprehensive psycho-social assessment, management, and support to at risk patients and families and will work closely with the providers, registered dietitians, and day staff to provide high quality stabilization and treatment for diabetic pediatric patients. The Social Worker's primary responsibility is to function as a consultation and referral service to address the psychosocial needs of the patient population served by the outpatient Joslin Pediatric Center. The Social Worker contributes to the achievement of optimum health, satisfaction, and cost-effective care by addressing the social determinants of health which may impede care.
MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
3 years relevant post-master's experience in a hospital and/or healthcare environment preferred.
Monday-Friday, days. Occasional on-call/holiday coverage on rotation, based on operational needs.
